  Consent To Participate in an Online Evaluation of the Marquette NFP Web Site

1. Invitation to Participate
You are invited to participate in a research study evaluating a natural family planning (NFP) web site. The NFP web site has many unique features such as an interactive online fertility charting, downloadable charts, user manual, completion certificates and other important instructions, and an interactive discussion forum where your questions will be answered by professional nurses and doctors trained in the Marquette Model of NFP. We also have expert theologians with a background in bioethics that will be posting in the forum. All questions will be screened by the professional NFP nurses within 24 hours after you post it. A question for the NFP nurse will be answered in the appropriate discussion forum within 24 hours after it is posted by the user.
2. Purpose
We are evaluating the Marquette University NFP web site in order to see whether it provides you with the necessary information to use NFP, to determine your satisfaction with using the online web site, and to obtain open ended feedback. The information that you provide will be used to improve the web site. Information entered on the web site (including online charting and forum posts) will also be analyzed for research purposes.
3. Description of Procedures
Participation in this evaluation consists of using the web site and the online charting system. You will be asked to complete a 10 item true false quiz and complete a 10 item user satisfaction survey at 1 month, 3 months, and 6 months of use. We would appreciate any other feedback you would like to provide to improve the online web site. You are free to use the web site, the information it provides, the online charting system and participate in the online discussion rooms as often as you wish. We would like you to use the online charting system for at least six months but you are not obliged to do so. You can use the web site as much or as little as you wish.
4. Risks and Inconveniences
There are no side effects in using this web site and charting your fertility to either achieve or avoid a pregnancy. The amount of use of the web site is up to you. However, we strongly recommend that if you are using the information and charting system to either achieve or avoid pregnancy that you follow the instructions that are provided and that you are consistent in your charting and observations of your fertility signs. You should know that there is a small risk of achieving an unintended pregnancy (i.e., 2% or less) if you follow the instructions and are consistent in your charting. If you do become pregnant, we will ask you to complete the online pregnancy evaluation form and to consult with the professional nurse NFP teacher. Please note that you will most likely become pregnant if you have intercourse during the fertile phase as indicated by the electronic fertility charting system.
5. Benefits
The benefit of participating in this online evaluation of the Marquette NFP web site is that you will have free use of the electronic charting system, the discussion rooms, and advice from the professional NFP teachers, physicians, and bioethicist. After the pilot study period, the user forums and consultation parts of the web site will only be accessed by a paid subscription. Another benefit of using the web site is the information it provides about monitoring and interpreting fertility. You can also have the satisfaction that you will be contributing to the development of an online NFP web site that will be used by many couples throughout the country and world.
6. Confidentiality
As you know, after you registered on this NFP web site you were provided a user name that will be yours for the entire 6 months of use. The professional NFP nurses, physicians, and bioethicist do not have access to your personal identity. They only know you by the assigned user name automatically generated by a computer not linked to Marquette University. The information that you provide us (i.e., the results of the NFP quiz and the user satisfaction) are not linked to your personal identity. Information that you provide will be used to improve the web site. We will also be presenting the data provided in aggregate form at professional conferences and in professional journals. The host server for this online web site is password protected and has a high level of security. The host server is a national corporation that hosts thousands of web sites and was selected by the Marquette University Information Technology Office as a secure host server.
7. Voluntary Participation
You do not have to be in this evaluation study if you do not want to. If you agree to be in the study, but later change your mind, you may drop out at any time. There are no penalties or consequences of any kind if you decide that you do not want to participate. You can stop using this web site and charting system anytime you wish, however, we would appreciate you completing the discontinuation form to help us to understand the reasons for discontinuation.

This office has a group of people that reviews research studies and protects the rights of people involved in research.
